Bring the printed form with your identity, Social Security, Texas residency, and course documents — for first-time applicants ages 18–24, that includes the Certificate of Completion from the Texas adult drivers ed course.

Full Legal Name
Use your name exactly as it appears on your birth certificate, passport, or other proof of identity. Do not use nicknames.
Date of Birth
Use the date of birth shown on your identity documents.
Social Security Number
Enter your SSN if you have one. If you do not have one, bring the DPS-required documentation explaining your status.
Contact Information
Use a phone number and email address where DPS can reach you about your appointment or license.
Address Information
Texas Residence Address
Use your physical Texas home address. A PO Box is not accepted as a residence address.
Mailing Address
Use this only if mail should go somewhere different from your residence address.
County of Residence
Enter the Texas county where you live, such as Harris, Travis, Dallas, Bexar, or Tarrant.
License Application Details
Application Type
Follow the DPS form labels for the license or identification card transaction you are requesting. First-time adult license applicants usually complete the original driver license application path.
Previous License History
List any state or country where you have held a driver license. DPS checks license history, so answer accurately.
Citizenship or Lawful Presence
Answer based on the documents you will bring to DPS. Non-citizens should bring the required lawful-presence documents.

Adult applicants use DL-14A. DL-14B is the minor form and can send you to DPS with the wrong paperwork.
